| The low water cut-off of a steam system is
tested when the burner is cleaned. We recommend that this
control be flushed periodically but assume no responsibility for
its operation. |

| Protection
Plan policy provides for the repair or replacement and labor
of any or all the parts listed above. No other parts
of the furnace, boiler or heating system are covered, such
as the servicing and replacement of any device that water
passes through such as piping, cartridge circulation 007,
zone valves, radiation, valves, domestic hot water hook-ups,
hot air dampers, & controls, etc. and labor cost to
install. | |

Terms and Conditions: This agreement does not
include: |
|
Parts or labor required as a result of
abnormal conditions such as water damage, fire, flood,
freezing, war, hurricane, or other such acts of God, power
failure, fuel run-outs when account is in arrears, failure
to check water level cutoff, damage or loss resulting from
delays or failure to render service for reasons beyond our
control and/or customer negligence.
Oil tanks
containing water, sludge, or heating units having excessive
previous service.
We will not be liable for damages
resulting from heat failure in a vacant or unoccupied home,
whether it be burner failure or lack of fuel, as vacant
property should be checked daily by owner or owner's
agent.
Coverage under this plan is available only for
residential heating units firing less than three gallons per
hour.
All fuel oil requirements must be purchased
from MARANDOLA Fuel Service during the term of agreement
under the automatic delivery system and paid for under the
credit terms established in our credit agreement. This plan
will be in effect for one year from invoice
date.
MARANDOLA Fuel Service reserves the right to
reject this agreement upon inspection of the heating system
up to 30 days from the invoice date.
The invoice sent
and paid constitutes final agreement by both
parties.
All equipment is to be inspected and
approved by one of our authorized service people before the
plan goes into effect.
All replacement parts are
subject to availability.
This plan cancels and
supersedes all prior agreements between the parties for
burner service. The plan will be in effect for one
year and will be renewed annually, subject to MARANDOLA Fuel
Service's right to modify the plan as to price or other
terms and conditions effective on the anniversary
date.
Any service required after 4:30 weekdays or on
Saturdays, Sundays or holidays must be the result of
complete mechanical failure of the unit. Any other
service call received during this time period will be billed
at the hourly rate in effect at that time.
Price
covers one (1) heating zone and standard control systems.
Additional zones and other conditions will be subject to
individual price quotations.
MARANDOLA Fuel Service's
liability is limited to the reasonable value of the service
and parts offered in each type contract not to exceed
$1,000.00.
MARANDOLA Fuel Service makes no
warranties express or implied, including but not limited to
any warranty of merchant ability or fitness for a particular
purpose except as expressly set forth herein. No
warranty is made as to the ability of the system to supply
heat or hot water required by the customer.
Service
work performed that is not covered by the Service Contract
will be billed to the customer at the current hourly rate*
per hour, plus parts.
Normal working hours are 8 a.m.
to 4:30 p.m., Monday through Friday, Holidays
excluded.
Routine maintenance will be performed only
during normal working hours.

| Termination: |
| The agreement may be terminated by either
party at any time. If this agreement is terminated, the
charge for any work done and the length of time that the contract
has been in force. |

ANY SERVICE CALL NECESSITATED BY THE CUSTOMER'S
FAILURE TO MAKE THESE IMPORTANT CHECK, WILL BE CHARGED TO
THE CUSTOMER AT OUR NORMAL RATES. |
